Approved Residency Training Programs

Residency Training Program Registration

Each specialty has a Residency Training Committee that specifies training criteria that must be met for certification. This committee will review training programs submitted by institutions and residents, and rule on their acceptability.
All residency training programs must have a designated Program Director. (See section D.2.d of the General Information Guide) The Program Director is responsible for submitting the original residency training program registration form and updating the form each year. Program update forms will now be sent to Program Directors by January 15 and are now due each year by March 1.

Residency Training Program Approval

All residency training programs must be approved prior to any residents starting the training program.

Directions for completing the Program Registration Forms

  1. Select the form you would like to open from the list below.
  2. You will be asked if you would like to open or save the file.  Select Open File.
  3. The form will open in Microsoft Word.  You can enter text in the appropriate fields, and check boxes for the appropriate categories.
  4. After completing the information, select File, Save As and Save the file to your hard drive.
  5. E-mail the completed file to: Ron@ACVIM.org for Cardiology, Large Animal Internal Medicine, Neurology, Oncology, and Small Animal Internal Medicine.

 

Residency Training Program Registration Forms (due by Program Directors annually)

 

**Please note, any candidate that significantly changes or alters their residency training program before completion must notify ACVIM, in writing, before the changes are made to ensure that the proposed changes are approved. Significant changes could include, but are not limited to, transferring from one program to another, alterations in program duration, beginning another Residency training program, or enrolling in an institutional graduate program.

Authorized Agent Letters (due by Program Directors annually)

An Authorized Agent Letter must be completed and submitted along with the Residency Training Program Registration Form.  The Authorized Agent Letter must be signed by the Department Head if the program is a University Residency, or an Owner or Officer of the company if the residency is in a Private Practice.  If the Program Director has any questions about the most appropriate individual to sign this form they should contact the Chair of the Specialty's Residency Training Committee or the ACVIM Central Office for clarification.


Candidate Registration

Once a program is approved, residents must submit the ACVIM Candidate Registration Form within 90 days of beginning their residency training program. The purpose of registration is to ensure that a candidate embarks on a training program that conforms to the requirements of the specialty in which he or she wishes to become certified. Failure to register, or registration after the 90-day deadline, may jeopardize the resident's certification process, as some of the training program may not be recognized or accepted.
